Rectangular bronze plaque with raised edges. A woman in a long dress kneels on the left, reaching out to a bust of a man on the right. She wears a laurel wreath on her head and holds a branch in her left hand while touching the bust with her right. The man depicted in the bust has short hair. In the top left, several putti fly amidst clouds. A tree trunk with leaves is on the right side. A mountain landscape with trees is on the left. The text "GRENNBLE ET LA COTE SAINT-ANDRE A HECTOR BERLIOZ" is engraved above the kneeling woman. The background of the plaque fades to lighter bronze. The background behind the plaque is grey that gets darker towards the top.
