Beige-colored, aged paper with darker edges, stained and discolored in places. Two red vertical lines, one on each long side, extend from top to bottom, with the left line thicker than the right. Small piece of paper on left side with handwriting. Text written in black ink with some red ink. A large red dot centered, roughly one-third of the way down. Small text in top-right corner.
