A bronze-colored medal with a raised image of a man in a military uniform with epaulets and a high-collared jacket. The man's head is turned to the right; he has wavy hair and a mustache. Around the edge of the medal on the top half is text that reads "OLIVERUS H. PERRY, PRINCEPS STAGNO ERIE" around an image of the man's head and shoulders. On the bottom half, there is more text that reads "CLARISSIM TOTAM CONTVDIIT". The medal sits on a surface that fades from dark at the top to light at the bottom. A shadow is cast under the medal.
