Yellowed, aged paper with darker edges. Pale red lines forming a large rectangle cover most of the page. The rectangle has 10 evenly-spaced horizontal lines across the left two-thirds. A vertical line down the right side of the rectangle intersects the horizontal lines. The paper has several darker spots, a large stain at the top right corner with faint black writing and several dark smudges on the bottom right. The bottom right corner shows a small pencil-written number. The page edges are rough. The right edge appears to have several small, darker spots. The lower right corner curls under. The left edge of the paper is bound to another piece of paper.
