The top of the medal has a raised inscription that reads "IN COMMEMORATION OF THE HUNDREDTH ANNIVERSARY OF AMERICAN INDEPENDENCE". The center of the medal depicts a woman in a long dress standing and holding flowers or a wreath. She is surrounded by three other people, two women and one man, all dressed in 19th-century clothing. The bottom of the medal has the year "1876" and another line of text that reads "ACT OF CONGRESS JULY 4TH 1875". The background of the image is light gray with a shadow cast by the medal.
